To request an assessment/evaluation of your preschool age child, please complete the following:

1. Click here to enroll your child(ren).

             You will need to upload the following documents:

• • Immunization records
  • Original Birth Certificate or Passport
  • Parent/Guardian picture ID
  • Proof of Residency (Burbank Water & Power bill (preferred), Gas Company bill, Landline Phone bill (cell phone bill not accepted). If the bill is not in parent/ guardian’s name, the person on the bill will have to write a letter of residence. The letter must include parent/ guardian’s name, child’s name, date of birth, address, and contact numbers.
