Please be sure your NAFTrack progress screenshot displays your internship progress with green check marks.  

You MUST write your full name, 9-digit ID, school name, academy name, supervisor name, and supervisor email at the top of the form BEFORE you present it to your supervisor and upload it to your portfolio page.
