PLEASE NOTE: 

• Schools may no longer accept any type of written statement from a licensed physician. Instead, physicians must complete a standardized exemption form that contains specified information, and they must submit those forms electronically, both to the school and also the California Immunization Registry.  Unless a standardized medical exemption form is on file, schools may not admit students who are not fully immunized on the basis of a medical exemption. 
• Exemptions based on temporary medical conditions will continue to be valid for only one year. Exemptions based on permanent medical conditions will be valid for only one grade span.  When a child with a permanent medical condition completes one grade span and seeks to enroll in the next, physicians must complete a new exemption form.
• The California Department of Public Health will create a monitoring system that includes review of schools whose overall immunization rates are less than 95 percent; schools that do not provide annual reports of vaccination rates; and physicians who submit more than five medical exemptions in one year, starting January 1, 2020.

Medical Exemption Information

MEDICATION OF ANY KIND, INCLUDING OVER THE COUNTER MEDICATION AND TOPICAL MEDICATION, MUST BE ACCOMPANIED BY A MEDICATION ASSISTANCE FORM. PLEASE USE ONE FORM PER MEDICATION.

HEALTH EXAM REQUIREMENT

A health examination is required by State Law for first grade students. This health checkup is recommended at the kindergarten level but not earlier than 18 months before admission to the first grade.

This examination includes a nutritional history, a complete physical examination, tests for anemia, urine analysis, tuberculosis screening, and vision and hearing screening. Necessary immunizations may be obtained at the same time.

You may comply with the law in one of these ways:

• Visit your doctor and ask your doctor to complete the attached “Report of Health Examination for School Entry” and return the completed form to your school upon enrollment.
• If you are a Medi-Cal beneficiary and do not have a private doctor, you may call any of the clinics or doctors on the attached “Providers List” for an appointment. Low income families whose children participate in the free school lunch program, may qualify for free health examinations.
• If for some reason you do not wish your child to receive the required examination, please consult the nurse assigned to your school site to discuss a “Waiver of Health Check-Up for School Entry” form.

Your child may be excluded from school as of December 1 of the first grade year, unless we receive documentation that the above

requirement has been met. For information on required immunizations, please visit www.shotsforschool.org.

ORAL HEALTH ASSESSMENT

To make sure your child is ready for school, California law, Education Code Section 49452.8, now requires that your child have an oral health assessment (dental check-up) by May 31 in either kindergarten or first grade, whichever is his or her first year in public school. Assessments that have happened within the 12 months before your child enters school also meet this requirement. The law specifies that the assessment must be done by a licensed dentist or other licensed or registered dental health professional.

Take the Oral Health Assessment/Waiver Request form to the dental office, as it will be needed for your child’s check-up. If you cannot take your child for this required assessment, please indicate the reason for this in Section 3 of the form. California law requires schools to maintain the privacy of students’ health information. Your child’s identity will not be associated with any report produced as a result of this requirement.

Here is important advice to help your child stay healthy:

Take your child to the dentist twice a year.

Choose healthy foods for the entire family. Fresh foods are usually the healthiest foods.

Brush teeth at least twice a day with toothpaste that contains fluoride.

Limit candy and sweet drinks, such as punch or soda. Sweet drinks and candy contain a lot of sugar, which causes cavities and replaces important nutrients in your child’s diet. Sweet drinks and candy also contribute to weight problems, which may lead to other diseases, such as diabetes. The less candy and sweet drinks, the better!

Baby teeth are very important. They are not just teeth that will fall out. Children need their teeth to eat properly, talk, smile, and feel good about themselves. Children with cavities may have difficulty eating, stop smiling, and have problems paying attention and learning at school. Tooth decay is an infection that does not heal and can be painful if left without treatment. If cavities are not treated, children can become sick enough to require emergency room treatment, and their adult teeth may be permanently damaged.

Many things influence a child’s progress and success in school, including health. Children must be healthy to learn, and children with cavities are not healthy. Cavities are preventable, but they affect more children than any other chronic disease.

If your student has an injury that requires a temporary exemption from Physical Education class (PE), please have your physician complete the form below and submit it to your school nurse's office.

PE MEDICAL EXEMPTION FORM

Students returning to school following an injury MUST have written authorization from their doctor/health care clinic allowing them to wear or use the following:

• Cast
• Crutches
• Sling
• Brace
• Ace bandage
• Sutures
• Ongoing wraps or braces for PE

The doctor/health care clinic must also state any restrictions necessary for the safety of your child while at school.

Anyone exposed to people or animals with Mpox should monitor their health or be monitored for signs or symptoms consistent with Mpox for 21 days after their last exposure. Individuals exposed to the Mpox virus can continue their routine daily activities (e.g., go to work or school) as long as they do not have signs or symptoms consistent with Mpox.

RE:                  Fragrance Free School Environments

In order to follow Board Policy and ensure good indoor air quality, we would like to remind everyone that fragrances and infusion products can cause allergic reactions, breathing difficulties and trigger other medical conditions for some children and adults.

Per our Board Policy and Administrative Regulation 3154, staff and students are asked to refrain from bringing common irritants such as the following:

• Furred or feathered animals
• Stuffed toys that may collect dust mites
• Scented candles, incense, or air fresheners or diffusers
• Perfume or cologne, scented lotion or hair spray, nail polish or nail polish remover
• Other personal care products that are not fragrance-free in classrooms or other enclosed areas or buildings.

We know that, in close environments with many people, certain natural smells may occur. Unfortunately, efforts to cover up naturally occurring odors may cause someone’s asthma or allergies aggravation.

We appreciate everyone’s efforts to assist us in this matter.
