    John Burroughs uses an online clearance process. Most of the paperwork required in the past is now completed through Home Campus’ athletic clearance. The only exception is the Physical Examination Form.


    There are two steps for an athlete to clear for participation in summer camps, athletic practices or try-outs:

    1) New Athletes – Go to www.homecampus.com/login. Set up a user account and password. Complete the information (YEAR 2024-25 / SCHOOL BURROUGHS/BURBANK-SS) and electronically sign all required forms. Be aware that you will need emergency contact information and proof of insurance to complete the process. *


    2) The Official Physical Exam Form must be used. It should be completed and signed by a parent, signed and stamped by an MD and uploaded to the Home Campus account. Incomplete forms will be denied until required document is submitted.


    *Note: Medical Insurance is required to participate in High School Athletics. Any Athlete without medical insurance must purchase School Insurance at a minimal cost. There is a link to purchase School Insurance embedded in the athletic clearance process. You may also visit the Aldrich Taylor Insurance website and purchase the insurance on-line (BurbankStudentAccident.com). Please bring verification of purchase to the first day of practice. Again, this is only required if the athlete does NOT have medical insurance.
